- Can the applicant live independently with minimal assistance?

- What city or county does the applicant currently live in?
- Is the applicant a veteran?
- Is the applicant a senior citizen?
- Is the applicant currently experiencing homelessness or serious housing instability?
- Is there any additional housing or support information we should know?
