  2. Is the applicant at least 18 years old?
  3. Can the applicant live independently with minimal assistance?
  4. Does the applicant currently have income or financial support?
  5. Is the applicant currently working with a case manager?
  6. What city or county does the applicant currently live in?
  7. Is the applicant a veteran?
  8. Is the applicant a senior citizen?
  9. Is the applicant currently experiencing homelessness or serious housing instability?
  10. Is there any additional housing or support information we should know?
